At SP Therapy Services, we understand the importance of structured, timely, and clinically robust reporting for case managers and legal teams. Following an initial neurological physiotherapy assessment (typically within 2 weeks of referral), we provide a comprehensive report outlining assessment findings, a detailed treatment plan, and recommendations for the first 3 months of rehabilitation.
Each of our Physiotherapist reports includes clearly defined SMART goals, alongside a transparent breakdown of associated costs to support funding approvals and ongoing case management.
Each of our initial Physiotherapy Report and Updates Treatment Plans contain:
- Description of the current rehabilitation need
- SMART Goal
- Functional Related Aim
- Provisional plan of Physiotherapy needs for 3 months
- Clear associated costs
To ensure continuity and accountability, we provide regular 12-week progress updates, including revised treatment plans and measurable outcomes aligned to your client’s rehabilitation goals.

- Are your reports and treatment plans peer reviewed?
Yes we have a robust internal peer review process for all documentation leaving the practice which aims to both maintain and improve our Practice standards.
- What happens if the patients need exceeds the treatment plan?
We will e maintaining regular contact with you as the case manager and if the patient’s rehabilitation needs change from those identified in the treatment plan we will provide a detailed Additional Information document explaining why and any associated costs.
- Do you provide P35 or expert witness reports?
No, our focus is as treating clinicians.
